Where to Access Your Tax Forms (W-2, 1095-C, 1042-S)

Your annual tax forms, including your W-2 Wage and Tax Statement, are available electronically through Paylocity. To view or download your forms:

• Log in to the employee portal at https://access.paylocity.com/

• From the Self Service menu, navigate to the “Pay” section, click “More”, then “Tax Forms” section in the menu.

• Your tax forms will be available to view and download as a PDF by January 31st each year.

• If you have previously consented to electronic delivery, you will receive an email notification when your forms are ready.

If you prefer to receive a paper copy, please update your delivery preference in the portal or contact Human Resources by January 15th. If you have not provided electronic consent, a paper copy will be mailed to your address on file.
